
Home Safety Evaluations
Whether you are facing a progressive illness, a sudden medical condition, the challenges of a recent accident, or simply seeking solutions to make your home more accessible and functional, we are here to help with a Home Safety Evaluation.
Our customized home safety evaluations are conducted by a licensed Occupational Therapist either virtually or in-person. The Occupational Therapist will conduct a thorough assessment of your (or your loved one’s) home to identify potential fall risk areas, barriers to activities of daily living, and the overall accessibility of the environment.
Areas assessed include points of entry, bathrooms, kitchens, bedrooms, and indoor/outdoor living areas. Our experienced therapist then recommends tailored modification solutions for safe, comfortable, and independent living to meet each individual’s unique needs and goals.
An experienced Occupational Therapist conducts all evaluations.
Evaluation Options

Standard Evaluation (Virtual)
Based on observations of your environment via videos/pictures and an interview of the individual. Our therapist will make recommendations for improving the home environment and the individual's ability to perform daily activities safely and independently.
This can include recommending specific assistive devices, basic home modifications, and/or providing training on the proper use of equipment or adapting tasks to accommodate the individual's abilities.
This is a great option for those looking to:
· Return home from the hospital or have recently returned home from the hospital.
· Recover from a recent injury/illness
· Identify and address basic safety concerns
· Make modifications to decrease fall risks
$149 for 60 minute virtual assessment

Advanced Evaluation (In-Person)
A comprehensive, performance-based assessment focused on how the individual interacts with his/her environment. It includes all aspects of the Standard Evaluation plus observing the individual perform actual or simulated daily activities such as bathing, dressing, bed mobility, cooking, climbing stairs, going in and out of the house, and other functional tasks necessary for independence and well-being. Recommendations may include extensive modifications (ramps, widening doorways, stairlifts, and adaptive equipment), ergonomic adaptation suggestions, collaboration with contractors, and a detailed action plan for implementing changes. The therapist will also look for areas where the individual may need additional support, such as mobility, fine motor skills, fall prevention training, or cognitive function, and make appropriate referrals.
Recommended for individuals with significant loss of function due to:
· Stroke
· Orthopedic conditions
· Arthritis
· Amputation
· Brain or spinal cord injuries
· Progressive illnesses such as Parkinson's Disease, Multiple Sclerosis, Degenerative Joint Disease, Rheumatoid Arthritis, Dementia, Macular Degeneration, etc.
Also a suitable option for individuals looking to:
· Remodel their home/bathroom to make their environment wheelchair accessible
· Make significant changes to their home to improve accessibility
Consists of an in-person visit of up to 90 minutes and a 30-minute follow-up phone call after recommendations have been implemented.
